Insurance
Medicare and most private insurance companies are accepted. Please remember to bring your insurance cards to each visit so that we may file your insurance.

Fees
Co-payments, deductibles and co-insurance amounts are due at the time of service. We accept VISA and MasterCard. If you should need to make special arrangements concerning your bill, please call the office prior to your appointment.

Please familiarize yourself with our cancellation policy. Failure to cancel an appoinment in a timely manner may result in additional charges.
